
Every day, the Health on the Streets van travels across the Central Coast, meeting people where they are. From parks and car parks to laneways and beaches, our team provides health care, support, and a listening ear to those doing it tough.
Stories from the Van shares real moments from our outreach. Snapshots of resilience, connection, and hope. These stories highlight the strength of people experiencing homelessness and the compassion of the community around them.
From small wins like a toothbrush or being heard, to life-changing steps like safe housing or better health - this is what’s possible when care goes beyond clinic walls.

Genuine Care That Counts
“For me going out into the community is such a privilege because it means that we get to go out to where the people actually live: as a peer worker, I get to bring the human side to our work. To be able to share in someone’s grief, or to celebrate their successes is the utmost privilege. For many of the people we meet, there is a sense of discouragement, that everything is too hard, they don’t know who to turn to and have feelings of rejection, most have experienced significant grief and loss, or other traumatic events.
To be able to reassure people that they are not alone, and that there is someone in their corner, who has lived experience and believes in them, that can acknowledge that it is a tough road but we have a lot of connections within our community and will advocate for them. As a peer worker I get to help people to know that there is hope, and a chance to make life worth living. It can turn someone who has given up, into someone who is willing to engage with our nurses or re-engage with social supports and to try to live with dignity and quality of life.”
